Cumulative Preventive Care Bonus

The Cumulative Preventive Care Program is one of the cornerstones of Primary Care Renewal as it partners with family physicians in the Patient Enrolment Models to reduce the negative impacts of preventable disease through prevention, screening, and education. The Ministry of Health has implemented the Cumulative Preventive Care Program to support family physician in delivering preventive screening services to their enrolled patient population.

The Ministry of Health issues physicians in Primary Care Enrolment Models (FHG, CCM, FHN and FHO), biannual Target Population and Service Reports (TPSR). These reports provide data on their enrolled patients who have/have not received specific preventive care services.

Primary Care Advisory works with a comprehensive in-office training model to teach staff how to correctly complete a TPSR Previous Report that is complete and only requires your staff to calculate the bonus and submit the claim. By working with the chart below you can establish an in-office Preventive Care tracking system where you submit tracking codes for only those services for which OHIP does not receive claims data.

The in-office training session addresses the following for each of the five preventive care categories:

  • Outline for the criteria for establishing the Cumulative Preventive Care Bonus thresholds,
  • Outline how to maximize achievement thresholds based on information provided in reports from the MOH, Ontario Breast Screening clinic, Cancer Care Ontario and the Ontario Cytology Date Base,
  • Specify the number of patients whose charts need to be reviewed to confirm their eligibility for a specific preventive care service within the target dates, and
  • Review the physician’s office procedures for managing the Cumulative Preventive Care program to ensure that preventive care services are being tracked appropriately so that future MOH Target Population and Service Reports will provide all relevant information on preventive care services received by registered patients.

Where the MOHLTC documents identify that the physicians’ Cumulative Preventive Care achievement levels are significantly below the minimum billable threshold, we will provide the physician and their staff the information and education required to ensure their ability to participate in the Cumulative Preventive Care Program in future years.

Our program also includes information and training on collecting all of the Primary Care Premiums and Bonuses in the various Primary Care Patient Enrolment payment models (CCM, FHG, FHN and FHO).
