Other Primary Care Models

The Family Health Group (FHG) and Comprehensive Care Model (CCM) are payment models under Ontario’s Primary Care Renewal program that remunerate in addition to the Fee for Service payment model. Both the FHG and CCM models act as an incentive by directing additional funding to family physicians who are providing comprehensive primary care to their patients and emphasizes illness prevention. Both models support patients ‘enrolling’ with their family physician as a means of strengthening the relationship between the patient and their doctor and as a means of tracking patients’ use of additional health services beyond the care of their family physician.

The Family Health Group is similary to a Family Health Organization in that three or more physicians may form a FHG to offer their patients additional services such as extended after hours clinics, vacation time coverage and access to the Telephone Health Advisory Service (THAS). The main difference between a FHO and a FHG is the capitation payment model under the FHO. For more information, please see the Family Health Organization page.

The Comprehensive Care Model is designed specifically for solo primary care physicians to encourage them to provide comprehensive primary health care to thei patients and to reward them for doing so. These physicians offer comprehensive primary health care services to their enrolled patients including regular office hours plus one three hour block of After  Hours clinic per week. Compensation is based on individual fee-for service billing with incentives and enhanced fees.
